    I have recently started using this generator: http://www.stanford.edu/~marke/npc.html It not only gives you prepared spells, but also gear, and the feat and stat choices are pretty sensible (there is a very very small chance your wizard will get power attack at random).

    Yes. Although I prefer it if I can build the adventure with some time to prepare, and not right at the table - I improvise enough as it is with the sidetrecks my players stumble into, or decide to pursue.

    From a DM's perspective, I ask the players what they want to do before I prepare the next adventure - or to tell me in advance if they want to leave the current region. I also check if they would like to travel to a region before detailing it.

    New fantasy settings; how do you explain to your players?

    I usually try to give the players as much of the information their PCs would know as soon as possible, especially when they create a PC native to the setting. Nowadays I compile the information in a file, with images, and mail it them. During play I deal out additional informations, if needed.
