Once per ray is definitely correct. However, to nerf the silly 2-level-dips in Warlock I'll houserule that a ray can only benefit from agonising if you have the appropriate level in the Warlock class. i.e. a Warlock 2/Socerer 18 gets 4 beams, but only one of those would receive +Cha. A Warlock...

Sneak attack triggers "once per turn". RAW that means it can also be applied on others' turns when using your reaction, so twice in a single round. Melee allows AoO, netting you tasty additional damage - at least occasionally, more reliably with a Dissonant Whisperer or Commander's Strike ally...

I think an important aspect is going to be if the game allows/facilitates DMs sharing their user-made campaigns. If it does allow that it'll be a big feather in its cap from many CRPG gamers who loved NWN. If not they're gonna be royally pissed and many will probably refrain from purchasing.

Sorcerers with the Subtle Spell Metamagic are obviously the best at this. I'd also allow a Sleight of Hands check to try and do Somatic components without others noticing.

War Caster is slightly specific, adding a new way to use your reaction AoO. Reach/PM interaction is much more specific, working with a handful of weapons and requiring a feat.

Specific Beats General, PHB 7. If we're going down this route please provide reference, book and page which says determining how to use a Reaction is a multi-step logic-free process.
