Have to second the Noble Knight suggestion. I got good (not great, but good) pricing on my 3e and 2e products before the release of D&D 4e, and bought a bunch dice, books, and minis with the credit I earned form Noble Knight. Friendly, easy to work with, and quick turnaround.

I've pretty much always used a sliding scale of failures for situations like failed skill checks. Fail within 5 or so? You can try again if time permits. Fail within 10? If you've got the time, are willing to waste even more of it, and possibly take a -2 penalty depending on the situation...

Regarding: A) Certainly party size/composition, but also keeping a good mix of brutes, soldiers, artillery, lurkers, and controllers so that the party isn't killing 5 of the same type of monsters who all happen to have the same or similar roles (unless a given encounter really calls for it)...

Ah, see, I just foster Grey and Gray Morality, and let my players fall on opposite sides of the same issues without any of them ever being truly right. I accomplish the same thing, with about zero effort. ;)
