From my players PoV the setting changes greatly. My current group has been in 4 of my campaigns, spread out across ~700 years. My first campaing had an "all-powerful" church, later campaigns dealt with its fracturing power.

I like to mix "real world" names with more fantastical names:
Real world names: Civil Government, Central Territories, Silver Sea, Araby
mythological name: Midgard, Polarion, Thule
fantastical names: Serpent Cliffs, Pit of Words, Castle of Storms
I was realy supprised when I saw the preview a few months ago that explained the plot... I'd though that it was just Samuel L. Jackson vs. alot of snakes.
I can easily see how a DM could ban the create wonderous items feat. There are alot of items in there, and in the hands of a clever player they can be game breaking (even more so when you start allowing players to create their own items, or start allwoing those from non-core sources)
Personaly I like dragons that are crafty and full of sorcerous power. It lets them do realy crazy things... like eat every left handed person in a kingdom, or return every 50 years to ravage a kingdom, then come one day early on the 51st year.

I've used the same campaign setting in all my D&D games (though one campaing took place on the other side of the world, and so was in many ways a "new" setting). Each campaing has taken place over a different area, though there has been some overlap, adding a bit more detail to the world.
I've always prefered the WH elves to the D&D elves, a much more interesting bunch IMO.

Started playing my first year in college at at the ODU Sci-fi and Fantasy club. Joined because they played Battletech then moved on to Westend Starwars before being invited to a D&D game by one of the other players in the starwars group.
i was 17, some time durrng the 97-98 school year (my first year in college), at the Sci-Fi / Fantasy club... I'd been playing Westend Starwars and one of the guys invited me to his D&D game (AD&D). We played for a good while and I had a blast, but the game broke up before 3rd arrived.
