    I'd go with a massive flat penalty: -1 spell slot per -1 of ability penalty for every spell level you can cast. It's brutally punishing, but at least it sucks evenly. -blarg

    As long as you make casting stat penalties remove spell slots appropriately, I think you're good to go.

    You can use any 3.5 WotC book? Mix in Book of Nine Swords. One level of crusader will give you access to the Iron Guard's Glare stance. Just leave it on forever, and any opponent adjacent to you takes a -4 penalty to attack anyone other than you. -blarg

    Trading module. Options for players to run caravans taking goods from one place to another. E6 module. Nuff said. Mage duel module. This design space could be deeply entertaining, especially when you consider school vs school, or even magic vs psionics.

    Healing surges are basically just a variant on reserve points. Both of those systems have interesting effects on gameplay, which is beneficial for certain campaign styles. As such, I think they'd both make fine additions to the 5e equivalent of Unearthed Arcana (and the 5e SRD as well). -blarg
