I would suggest talking to players. Get their input. Ask them what are they interested in doing, where do they wanna go, what are short term goals of their characters etc. That can give you some inspiration and also direction in which way to steer things.

I don't have particular number in mind. PC death happens as a mix of bad luck (on the players side), very good luck on DMs side, poor player decisions ( Leroooy Jenkins style will get you killed). I do have some house rules that make game more dangerous though.
