There are quite a few monsters where I'm going to want to write up some alternate sets of abilities. After the third or fourth lich the legendary/lair actions are going to get a bit stale.
"...a werewolf has heightened senses, a fiery temper, and a tendency to eat rare meat."
LOL. I saw a werewolf eating drinking a piña colada at Ruth's Criss...
Not really, no more than any other 3.0./3.5 monster book. It was very poorly bound and they did not offer replacements. So I finally caved and bought a second copy because it was starting to get ridiculous prices on the secondary market... then they released the Tome of Horrors Revised PDF for...
Without being there or being the DM, I don't know. I would resent a player running off and talking about it (and only presenting one side) on the net before talking (not arguing) about it with me first.
Too early to tell.
A strong point in its favor is that it is eminently readable, but still works well as a reference.
I think I'm waiting for 5th Edition Foes 2.0 Re-Revised this time around. :D

I think you're making up rules to win an argument on the internet, which is pretty cool. They need to match the prices and weights in the PHB (probably... ask your DM), and that's about it. Nothing about size.
I don't see the bias of rounded dice mattering too much unless you play bucket-of-dice games like Warhammer 40k (rolling 100 d6s is called "turn 1" for my Tau or Imperial Guard) or RPGs that use dice-pools.
Throwing aside dice bias, I love pine dice the best for their light feel and bounciness...
Y'know, I think I was mixing him up with MSB, Terry wasn't exactly a pit bull.
As for where he is now, wasn't he big into Chivalry & Sorcery? Does C&S have any sort of online fandom these days?
My problem with RAW, RAI, CLIntS or any of those acronyms is that they lead to a forum game of arguing rules that is completely unrelated to playing the game. GAP is king as far as am concerned.
