    I don't have it front of me, but there was also the 4e version of Tomb of Horrors, which was basically a sequel to 2e's Return to the Tomb of Horrors. I don't recall specifically if it was set in Greyhawk, setting-agnostic, or imported into the default Nentir Vale setting, though.

    It certainly shouldn't be. If the DMG doesn't tell DMs how to do it themselves- "doing it" including crafting their own monsters- it's not much of a guide for DMs at all. (With the caveat that if those specific rules are in the MM, that's probably okay.)
