I think it is going to vary a lot based on the actual cosmology, the specific magics available, the specific gods involved, and the history of the world and its cultures. D&D afterlife varies - in 4e the gods of knowledge and death do not know what happens to souls after they leave their period...

I've done the reverse. :) I played a (pathfinder 1e) sorcerer with the stars (mythos) bloodline where his noble family generations earlier had engineered a mythos encounter/event thing in their ancestral hall to develop a sorcerous bloodline among them.

According to the 2024 SRD kobolds are now base neutral and not evil. Hobgoblins are base LE. Goblins are base CN. Bugbears are CE. Drow, Gith, and Tieflings are not in the 24 MM (just some conversion notes for the drow), and the 24 PH entries does not say a lot about the drow and tieflings.

I would not say 1e assumes humans are LG at base. The 1e MM puts them all over the alignment map for different professions like merchants being neutral, pilgrims covering all five alignments (it was written post Holmes Basic five alignments but before the development of the PH nine alignments)...

I don't think that was quite it, I think for D&D they just wanted it to be more fantasical. They included a bunch of real world diseases in d20 Modern right in the core book and in the d20 Modern SRD so it was there to be used in gaming and it was easy to add into D&D if you wanted.
