I tend to let my players roll. But if they think they failed, like roll a one on a search and I try to hide the fact they found nothing, and start to look some more, I just change the DC on them to something around a DC 500000 :D
Perhaps, but if the defender is unable to addequitly defend himself (due to being flat footed or by being flanked), the Beserkers wild, reckless strikes are going to more than likely be able to hit those vital spots, and deal more damage...

I don't see why its a problem. They're expendiing XP from both Permanancy and Limited Wish. How hard would it be for the wizard to find another wizard to cast the spell on him?
It would make sense to me that you would have to. You gain the benifits of ghost touch by using the weapon, you gain the benifits of the various burst weapons by using them. Yeah yeah, I know their are exceptions (like the luck blade bonus), but these exceptions are spelled out.
