Imo the name Monster Manual is a sacred cow that needs to die. We need a lot more than monsters in there. NPCs, ways to interact with non-hostile creatures, traps, enviromental features all should go there. Make it the Challenge Manual or something.

Baphomet's war with Yeenoghu has been contained in the Abyss. Yeenoghu's cults were always gnolls living in savannas and Baphomet's cults were minotaurs in underground mazes. They never interacted in any way. So I go back to my original "Why did it have to mention Yeenoghu?"

Dragons routinely fly across the different world in my campaign setting. I have the feywild and the shadowfell as different planets not that far from the main world and dragons often fly between the main world and the feywild.

Well in my homebrew (retrofitted for some 4E concepts), you can actually dig deep enough that you fall off the world and down into the elemental chaos. Essentially there are no other infinities, everything exists in one universe.
