My first experience with them, as Maedar, that I recall was in Dragon magazine, probably early into 2nd edition. After that they started to appear in other monster books. They didn't have the snake hair or petrifying gaze though.

I'm not a fan of it but I'll just use the classic imagery. It's something I have noticed with this Monster Manual - changing established appearances, lore/creature types and genders when there is no need to. For me, I'll just stick to the classic imagery that has been D&D for decades.

For me, pretty much this. There are few and fare between high level adventures, and I've yet to find a full adventure path style campaign that goes 1st to 20th for 5th edition.

1st and 2nd edition we never bothered. I think that was because combats were less tactical in their execution. The D20 system changed all that and battle map with minis became a necessity. We've stuck with them ever since.

I'd say mid level is something like 6-12 but that does cover a wide possible range of power levels.
