Thresholds set by armor and level. Some special abilities also adjust it. If unarmored, it's (level)/(2×Level), no reduction uses. It's 0—3, actually. if damage roll ≤ 0, 0 HP if 1—(low threshold-1): 1 HP if (low threshold) — (high threshold - 1): 2 HP if (high threshold) to infinity: 3 HP...

The rules are pretty clear, at least to me: only a roll with fear gives the GM a free spotlight for the roll. A golden opportunity can also go straight to a spotlight, but that's not for the roll. Other spotlights take a fear, or masive player inaction. So NPCs are tied to either players...

The majority of western setting adventures are for Deadlands... and that's a weird west. There's a few seeds in the Challenge article for Space 1889 in the US, there are some adventure ideas in Western Hero... Most games that don't have compatibility with D&D lack adventures... at least ones...
