I don't play orphans normally, but it's generally rare for a character's family to be part of play at my tables, outside of modern-era games. I have had a few cases where two players ran characters with a familial connection, which is a nice bit of texture for intra-PC conversation.

Agree with this. I prefer when players make choices that drive the story and conflict forward, rather than chasing "immersion".

I view it as the players' job to bring functioning cars; my job as GM is to make sure the race track is clear and wave the starting flag. I GM my games a quarter-mile at a time. :)

And some us are just 5e customers, who view 2014, 2024, TotV and A5e as just different trims and colors of the same car model.

All of my in-person tables average around 3-3.5 hours per session, no matter who's DMing. I don't normally start at 1, but when it has happened, it's usually one session to get to 2, 2 sessions to get to 3, and then we hit our normal pace of about 3-4 sessions per level.

For me, yea, about that. Somewhere between 0.2 and 0.25 USD per page. (Or 4-5 pages per USD.) For $70, I would be expecting a 280-350 page book. I'd pay $60 for a 256 page book.
