    There are plenty of ways to represent three-dimensional combat. I've seen people use "stacked" battle maps to represent different levels. Personally, we have little disks that are put under the bases of miniatures to represent how high or low they are in battle.

    Yeah, but that's not a monster stat block, and PCs are not monsters. Again, if they have it (most don't, and I'm not sure anything does past MM1) it's in the stat block, as an encounter standard action.

    That's true. If a monster has a second wind it's specifically mentioned in the block. Bulettes are an example, so are liches and vampires from MM1. Vontarin, as it is, does not have that ability.

    Young Green Dragon's Instinctive Flyby

    Same here; that is how I played it when I ran it in an encounter. Since, as a solo, is intended to replace several monsters, only allowing the instinctive flyby to be used on a recharge cripples its action usage, and it becomes a lot less capable in combat.
