D&D 3E/3.5 3.5 Sunder/severing help needed

RAW is rules as written.

Malhavoc's book of iron might doesn't specifically have "severing" effects, but you can build any of the "handicaps" (hampering spellcasting, *temporarily* disabling a natural attack, blinding, etc) you wouldd like a target to suffer by using the robust "maneuvers" system presented in the first chapter.

Does Bastion's Torn Asunder book specifically treat mechanics for severing? That's a possibility as well.

This topic leads to a question I've always wondered about.... with no way to dismember an opponent, the only way to lose a limb is DM fiat. So... aside from the DM saying you've lost an arm to that nasty trap you didn't didn't disarm, what is the point of the regeneration spell?

Nylar.. That's actually what this whole thing was about. We found a scroll of regeneration (not randomly rolled, actually locked in a chest), so it had to do with something. But the DM says nothin in the quest we have to use it for (npcs, etc), and that it was for us. But why? Then the topic of severing came to mind and we couldn't figure out where to go. So apparently severing is a house rule and that's it.
