I've often wanted to try a druid/monk crossover, and with these stats, that might be wise. Try to get your highest scores in wisdom and constitution, take 8 levels of druid and one level of monk, and shapeshift into a tiger, a bear, or a dire ape (yell "Dancing Monkey Form!" before wildshaping for extra roleplaying points). Take natural spell, combat reflexes, weapon focus (unarmed), improved grapple, improved trip, dodge, mobility, spring attack, and maybe power attack or empower spell.

Pre-combat, make sure you've got barkskin up, and try to have mage armor up. If you've got a 24 wisdom (18 base, +4 item, +2 level advancement), then you'll have AC 15 (for the dire ape bit) +7 (for monk wisdom bonus) +4 (for mage armor) +3 (for barkskin), for AC 29, or 30 if you're dodging. Add in some dex-bonus items, or an intelligent magic item casting Magic Circle against Evil, or a simple Ring of Protection, and that goes even higher.

When you fight in this form, you want to be grappling and punching a lot. Keep up two Greater Magic Fangs at all times. In fact, a Prayer Bead of Karma would be nice for this, raising your attacks by +3 instead of +2. As a dire ape, assuming only the benefits herein, you should be attacking twice at +15 (+6 BAB, -1 size, +6 str, +1 WF, +3 GMF) for 1d6+9 (+6 str, +3 GMF) per hit, biting at +10 for 1d8+3 (unless you cast GMF on your mouth, which is overkill), and rending at 2d6+12 (including the +3 from GMF if your DM allows--this seems reasonable to me). You can flurry for three punches, but may want to be careful about that, since it lowers your rend chance. You'll have reach, so you can get a free attack on someone who comes into your reach; and your improved grapple makes it a great idea to try to grapple that person.
