What spell will take you from one planet to another in a Spell Jammer friendly cosmology?

Are different Planets still concidered one plane (the Prime)?

Ya, all planets are in the Prime Material plane assuming you are using stadard cosmology. However, I don't recall the spoelljamming rules, so I don't know if they made exceptions for this.

So the Spell needed would be Teleport, correct?

No, it specifical says in Teleport intra planatary travel is not possible. I think plane shift will work though

from the Concordance of Arcane Space:

"teleport: a charcter can teleport from one planet to another within a crystal sphere, but travel from one crystal sphere to another is impossible by means of this spell"

teleport without error adds - furthermore, the caster cannot teleport to another plane from the phlogiston

Nope - nothing can reliably take you from one crystal sphere to another except spelljamming. That was kind of the point. If you're lucky, you might find a magic portal which goes to an outer or inner plane, and another magic portal which returns to a different crystal sphere nearby, but they tend to be rare and ever-shifting.
