With what limitations would you allow wishing for ability to change into the fire elemental? (Medium - Large - Huge elemental, ability to change at will or fixed times / day)

Allowing something like this is probably out of the scope of a Wish spell. You could use Shapechange to do this... but certainly not as a spell like ability once a day type of thing.

A player who Wishes for this in my campaign would probably be transformed into a fire elemental immediately and permanently.

Mah... a Druid gets at 16th level the ability to shape into Small-Medium-Large elemental 1/day.

It really depends at which level you are wishing for this!

I would rather give it as a quest reward than just by casting a wish or buying a scroll of it. I have been thinking before of "greater wishes", and such an ability (1/day only, perhaps with a limit of, let's say, 5 minutes duration) is a good idea, possibly only up to large size, and fire only.

But definitely don't let it become a standard use for Wish, keep your DM's control over it, that is make the player feel it's a one time only gift, and with the limitation on time, size and type I won't feel the necessity of stripping the PC of extra Xp or other penalties.
 

Use your Wish to make the spell , Elemental Body [Fire] (sor/wiz 7, Manual of the Planes)...

...permanent on yourself as a supernatural ability that you can turn on or off as a free action.
 

I would allow one wish to act like a modified polymorph self: you turn into an elemental of size tiny to one size larger than you are, gain physical stats and natural movement. 1 hour/level. No (ex), (sp) or (su) abilities.

If a person wanted to use wishes to get this ability regularly, I would require 3. 1 for the poly, 1 for the permancy, 1 for the ability to supress this spell. Then again, I think wish is kinda stupid and would push them to do it via spell research, where it really belongs.
