Adapting the Dread questionaire to d20

My Friday night group has problems with character development past the cardboard-cutout. I'm planning on running the next campaign, and I think a Dread-style questionaire can help players get inspired and, perhaps, develop a back story for their characters.

What sort of questions would you put on a D&D, d20 Modern, or d20 Future character questionaire? Is there a public repository of questions I can use? To what extent should a question be able to give the player inherent perks or flaws? Has anyone done this before?

The Dread game is made so the questions fit the characters. So a bit about the characters has to be known first before the questions can be written. Also, the adventure has to be known so there can be questions that reflect what the group may be doing in that adventure. The Dread Questionnaire is very different then a generic D&D questionnaire that some people use.
