This is an idea I had for my homebrew setting, which is cut off from the rest of the cosmology... no inner or outer planes are accessible, no deities, etc, and only limited contact with outsiders.
The Ring of Souls is a massive cloud of people of all shapes and sizes orbitting around the world, hundreds of miles above the surface. Everyone intelligent being that's ever died on the planet exists up there, and the vast majority of them are in a sleep-like coma.
One of the things I've always disliked is the readily available nature of ressurection magic, so I usually ban it. It's not so much that I want the characters to die permanently, as I want NPCs to die permanently. However, I've come up with a concept of some people being "Fated" and the fates will actually bring them back.
Only the few "Fated" actually have a chance of coming back. All PCs are "fated." When a character dies, his player takes a draw from the Deck of Many Things. As long as he doesn't get The Skull (meaning permanent death), the character awakens... Soon thereafter, a spirit comes to him, saying it is not yet his time, and he begins falling out of the ring. He falls faster and faster until he finally lands in his body and awakens.
I want to give the other cards effects, too... Many of them would be bad, since I don't want ressurection to be free, but some would be good. For example, drawing Balance might ressurect them with the opposite alignment, The Void would send their soul back to their body but not ressurect it... They'd essentially still be dead, but their soul would forever be trapped in their corpse. The Jester and Fool would make you gain or lose 10,000 exp, respectively (no draw agains).
