OK, it's not in any books that I own, but if you put enough monkeys on a typewriter...

(and the loose/lose "issue" drives me utterly insane. Even worse than the ever popular rouge!)

Later in the module he was described as the very scared man, when I believe they intended it to be the very scarred man.

My favorite is in the cyclopes entry in my 2nd editon monster manul. It said instead of throws rocks for 4d10 damage, throws rocks for 410 damage. Ouch!

Didn't a later edition of the 2nd edition Player's Handbook have a search-and-replace error, substituting "mage" for wizard in certain spots?

As the tale goes, there were several spots in the printing where the term "damage" was replaced with "dawizard"...

However, I have not been able to confirm this.

Actually, I recall seeing this in one of the early Forgotten Realms products, the first product that was "2nd Edition" compatible. IIRC it was "Hall of Heroes" or the like. It contained the game stats of many of the Avatar series novel characters, among others. I specifically recall seeing "dawizard" in many places.
