Part of me wants to do a Devil-related epic book, whereby I take one Archdevil/Layer of Hell and then rework enough of the material to make an interesting adventure + monster book (in a similar way to my Vampire Bestiary).
My original suggestion for a (WotC developed) Monster Manual was to pick one theme every level and then develop 8-12 monsters around each theme.
So for instance you might have a "Palace of Orcus" theme with all the relevant monsters and NPCs. This way when you do 10 themes from levels 21-30 you would basically have a mini-campaign.
I think WotC touched on the idea, but tended to spread the stat-blocks across 30 levels. So rather than an Orcus entry with enough content to develop a Palace of Orcus setting, you get a bunch of low and mid-levellers as well as some epic tier henchmen.
When (IMHO) it would have been much better having a generic Evil Cultist theme for the lower levels (of a Monster Manual) and then give an Orcus theme the full epic treatment.
I've tried to integrate the 'theme' approach in the Vampire Bestiary (but try to link multiple themes together) wherein we have a Cannibal theme + Vampires (of the Jungle) theme + Jungle Animals theme.
Next up in Temple of Death is the Assassins (Thuggee Cult) theme + Evil Cultists (of Kali) theme + Vampires (of India) theme.
Personally I much prefer the theme approach and if I was putting together an Epic Bestiary I'd probably follow along those lines rather than having just a disparate mix of epic monsters.
