Attacking into a grapple

I know that attacking into a grapple with ranged weapons has an equal chance of hitting any of the grapplers, but what about with melee attacks? What about if you are attacking a pinned character?

Gah, I hate the grappling rules.

There is no randomness for melee attacks. The opponent does lose all Dex bonuses to AC (including, of course, Dodge bonuses). Note that a typical houserule for the randomness of ranged attacks is to weight the values based on the sizes of the creatures involved.

Well, not necessarily an equal chance.

As I2K alluded to, the rules say you roll "randomly" to determine which target you strike; a lot of people rule that you're 4 times more likely to hit a Large grappler (who occupies 4 times as much space) than you are his Medium opponent. You roll, say, a d10 and on a 1-8, you hit the Large guy. It's random, but the chances aren't equal.

Well technically a large creatures occupies 8 times the space of a medium. However in general d&d seems to count 1 large = 2 medium in terms of equivalency (see teleport, etc).
