Well, since humans are not in the MM there is nothing that says how advancement works. It probably should be class only, but without it's hard to tell. As it's weaker then even the NPC classes and the only reason to do so is to increase one's BAB to unlimited levels, I'd allow it. If you really want to do that for your character, be my guest.
 

The main reason I posted that is to show that the rules were needlessly complex :-P

In the case of, say, an outsider (a 20th level monk, for example?), then it may not be that bad...+2 to all saves, 8+int skills, +1bab, d8 hp (if I remember correctly), and they would only need to use one level in order to get one more attack. This could also happen with monster race pcs, etc. etc.

If they had simply capped the attacks at 4, or used your 20 best levels, or something of the sort, then that would make things a lot easier (since they do it that way with monsters anyway). As it is now, fighters somehow get better at fighting at the same rate as wizards, and wizards gain fort save bonuses as fast as barbarians...this, to me, is not quite how it ought to be.
 

Page 7, ELH. A Titan with a BAB of +21 is still limited to four attacks with that weapon, no matter how high his BAB gets. Not sure if that answers your question though.

Pg. 30, MM. A solar is not.

And of course, that's the only core outsider with more than 20HD, I believe. Since Solars gave a 'foretaste' of epicness with weapons of more than +10 enchantment (+5 dancing vorpals) and mighty longbows of more than +4 strength mod, either ELH or MM is wrong.

Check the Return to the Temple of Elemental Evil as well. Imix gets 6 attacks with his flaming sword.

I would go with the ruling in ELH. RttToEE and MM were out long before the rules for epic characters were set in stone.

Doesnt really muddy the waters. Outsiders use the fighter's BAB so they get +1 per HD/level up to 20 HD (or 20th level). After that, they gain an epic bonus to their attack rolls.

Yes, of course. That's why they included rules for it... wait, no they didn't! You can't level up as non-classed because of that. The leveling up rules say nothing of this. They merely tell you to choose a class in which you want to level. Creature types are not classes, and therefore --> Does not compute.
