[BadAxe] HoHF:D - qualifying for Paragon PrC

I loaned my copy of Heroes of High Favor: Dwarves to a friend (*). Now I can't remember what it takes to qualify for the Paragon PrC (fighter/paladin). Could someone enlighten me?

Note: I'm not asking for the whole class, just what the prerequisites are.

(*) Who was it who said - never lend a book to a friend because you could end up losing both?

Alignment: as patron diety
Craft: 8 ranks armor or weaponsmithing
Knowledge (dwarves): 8 ranks
Feats: Sunder, Weapon Specialization (favored weapon of diety)
Special: lay on hands
