Ok i have a BBEG who is a female that worships a plague god and is causing plagues. What i need help on is this ...

1. PrC? She's a Barbarian 1/Druid 3 (PCs are about 4th level)

2. Items that will help spread a plague

3. She can be advanced to about 6th level.

- A magical instrument to control rats. The rats being good carriers. Flies and mosquitoes are good carriers too.

I'd shoot for cleric, and take the Affliction domain from Bluffside.

And then utilize some of the rules from MEG's Blight Magic.

I was just thinking about how cool an NPC like that would be just last night, as a matter of fact.
 

With (2-3?) 4th level characters and a sixth level BBEG, forget prestige classes and stick with core class combinations. I assume the BBEG has some minions?

Good flavor doesn't come from classes, it comes from good portrayal of the NPC.
 


Do you have the book of Vile Darkness?

If you want to go for a prestige class, there are several that may work for you. However, I do not think that your villian will meet the requirements of the more intresting ones. Even the most likely ones will likely require feats from the BoVD, so some retroactive modifications may be necessary.

Vermin Lord:
You will get some nice special abilities that are not over powering, and every odd numbered level advances a pre-existing spell casting class. However, you need to be able to cast a particular spell to qualify, and one level of Druid is not going to do it.

Cancer Mage:
The villian is pretty much a walking disease, but the requisites are for a higher level then what you propose (+5 base fort save is what a 6th level fighter has). But the up shot is that it is

Thrall of Demogorgon:
Not much of a disease theme, but some nasty powers, and you can easily qualify for it without dramatically over shooting your players capabilities.

Thrall of Jubilex:
Very gross powers, and with some modifications / name changing, you can have a rather villian that should fit your disease theme. The requirements will put you a bit beyond your ideal level target though.

Anyway, think very carefully before choosing any prestige class. Most of the ones I have seen require you to be a fairly high level, and unless your willing to settle for only one level of the class, then you may end up over shooting your players power level.

Yeah, but Base Save pre-reqs are easy to twink. In fact, a Bbn 1 / Drd 3 qualifies for the +5 Fort requirement quite easily.

For myself, I'd suggest Blighter as a possible PrC. It's from MotW and fits the whole plague/decay theme quite well.

I had a nasty thought about this one. One mediaeval European idea about dealing with the plague was to light Plague Fires, bonfires, oftewn burning aromatic woods and herbs, in order to drive the plague vapours away. (This site gives a very brief summary.)

Now imagine small bundles of corrupted herbs that when added to the extant plague fires, far from stopping the spread actually encourages it. Either make those breathing the smoke make fresh saves against disease, or perhaps give a penalty of the Fortitude saves for already present diseases!

Very nasty, and really subtle. She may even disguise herself as a plague doctor, trying to help the sick! ;)
