Follow up question: Has anyone actually used Barbed devils as the primary creature in encounters?

Did you mean Bearded Devils? The second module in the War of the Burning Sky has a couple encounters with Kashyk, a Bearded Devil. I did not use culmulative damage, but I also had 6 PCs at 3rd level....I also missed the requirement for a caster level check

He escaped to come back another day {and another...and, well, maybe he will show up again }

I see the ability as a sort of poison, and now that I know about the caster level requirement, I would HR an increase in the DC of the heal and caster check by 1 for each additional injury... which is what I do with multiple doses of poison.

The damage should continue until the character dies... which makes it a nasty ability.

Well, not exactly. I used two Bearded Devils once to distract from an approaching Hellcat. I used cumulative bleeding, which seemed to be in line with their CR.
