I'm going to go with "Bad Moon Rising" in The Great Old Ones adventure anthology. For full campaigns, I'm kind of partial to At Your Door.

A goodly number of the Pagan Publishing scenarios are long out of print.

The Auld Grump, Walker in the Wastes remains my all time favorite CoC adventure.
