Not currently playing D&D...

...but when I do, it's homebrew all the way.

My D&D game pulled up stakes last year after our group scattered to the four winds (work-related transfers and travel, starting a PhD program in another city, &c.) - I alternated GMing with another member of the group, and both campaigns were wholly homebrew.

I've been picking away at a new homebrew D&D setting just for fun, but right now I'm focusing on a couple of different HB Modern campaigns - our current group really isn't wild about medieval fantasy.
 

While there have been a couple times when I have played in "official" campaigns, I only run homebrew, and most of my friends run homebrew as wells.

The longest running "official campaign setting" that I ever played in was a GURPS version of Empire of the Petal Throne...
 

I'm currently GMing two Forgotten Realms campaigns, one Dragonlance campaign, two Greyhawk campaigns, one Spelljammer campaign, one Kalamar campaign, one Ravenloft game, one Mystara game, one Traveller T20 game and one Everquest game.

Yeah, that is a lot, but hey, what other gamer wouldn't do the same if all of his time was free time!

Edit: Oh yeah, I forgot to mention my Sliders/Tangents game and my homebrew.

I am currently running a World of Darkness chronicle :)

I am planning to run the Greyhawk Istivin campaign arc from Dungeon and I am preparing a couple homebrew settings and a campaign on Highpoint (DragonMech) for future d20 play.

For future WoD play I am planning a Vampire: the Requiem chronicle and a near future version of the current game.
 

Homebrew for the last 26 years. I gladly lift, borrow, and steal from sources I like, but it is my world and my multiverse. 3E gave me the chance to throw in a cataclysm and rebuild from the foundation up. Third party providors have given some the best things to borrow for my rebuilt world. It is good to play 3E.

I only do homebrew. I will gladly play in any setting, but I will only run homebrew. As a DM, I need to have input on the setting. To me, there is no point in running games in a published world. Part of this is because I need an outlet for my creativity. Part of this is because I learned how to game in the time before published settings.

It's not necessarily that I have a problem with published settings, but they often leave me little wiggle room to make the game my own. Plus, with the plethora of 'accessories' WOTC puts out, it's not feasible for someone with a life to keep up with all that 'official' material.
 

Homebrew all the way, but I take heaping helpings from other settings like Eberron, Lot5R, IK, FR, and DL. It's a hodge-podge setting, but that's the way I like it!
