Cohorts?

PCs are only allowed to have one cohort at a time and they must have the leadership feat to have one, right?

There's some text in the DMG here and there that suggest both possibilities.

You need the Leadership feat to attract cohorts.

Each time you take the Leadership feat, you can attract one cohort.

Monte Cook has written several times about how this is a popular feat in his private campaign, and that at least one of his players took it more than one time, to attract an additional cohort.

Wowzers. I had just logged on to ask the exact same question. Each time the Leadership feat is taken, one also gets more followers, correct?
