I do the same.

I would take it that the continuous damage "last dealt" refers to the last (i.e. most recent) round.

Also.

As long as it is "continuous" damage, I think we can see it as taken continuously no? If it's 6 damage/round, the PC is hurt like 1 damage/second, but for playability's sake the whole damage is counted at once, even if the character doesn't feel hurt once every 6 seconds :)

It is early in the morning. So, please don't mind me asking a dumb question.

Have we decided, yet, that the damage is cummulative from round to round? I mean say Jack the wizard gets hit with a melf's acid arrow the first round then gets hit with one the second, and third round. Does all the lasting damage from the previous round stack with the new damage dealt with a new Mellf's acid arrow? I am still not quite sure if we have decided that.

All of the continuous damage the victim has taken since his last round.

In your example, with MAA = Melf's Acid Arrow:

Victim: Acts
Arrow Dude: Shoots MAA #1 at victim, hits
Victim: Acts. Spells require Concentration check for MAA #1.
Arrow Dude: Shoots MAA #2. Victom also takes additional damage from MAA #1.
Victim: Acts. Spells require Concentration check for round 1 of MAA #2 and round 2 of MAA #1
Arrow Dude: Decides to make it more difficult. Casts Choke (from T&B) on the guy. This can also cause continuous damage. The victim makes their save vs. it, so no damage.
Victim: Now at -4 for any spell requiring vocal component. Now at -2 for spells without a vocal component. Spells require concentration check because of any remaining MAAs, but now at an additional penalty from the Choke spell.

That is how I would run it.
