D&D 3E/3.5 Consequences of a single change to AoO rules for 3.5/d20/Pathfinder?

I haven't tried it yet, but have researched the question enough to know there are GMs out there running it with limitd AoO, and even gridless - I think all RPGs are fairly easily modded, but especially those of the D&D lineage.

Just leave the original AoO - withdrawing from combat - so this includes a kobold running past your fighter to whack the mage. Generally what is considered "crossing a square" still applies, gridless or not. And then add in whichever AoO make sense to your group, or as few, and allow them to change their options and address concerns however possible in regards to mechanics. Generally always give them the extra d20 if it makes sense in the moment. Is my 2 1/2 cents...

I think this significantly reduces the power and value of being Large or bigger in size. Since that's basically a ton of monsters, there is an issue with the balance between monsters being a little off, as AoOs from reach are a big component of the value of such a size.
