I have this NPC in 2nd ed who has a Berserk Frenzy ability. How does that translate to D&D3.5e? Like the barbarian rage?

See the Frenzied Berserker in the Complete Warrior Hanbook.
+6 strength, -2 ac, +2 will save, 2 hp of nonlethal damage each round of frenzy, cannot stop until everybody is down, even friends, unless a will save DC 20 is made, start frenzy at the beginning of his turn if wounded in the preceding round unless will save DC 15+damage dealt, exhausted at the end of frenzy, duration 3+bonus contitution.
It's cumulative with rage
It's something like that.

Yup, pretty much. IIRC, you're just fatigued at the end of your frenzy, unless of course you also raged in the same time, in which case you take double fatigue and become exhausted.
