Death Penalty

What rules do you use for a death penalty? Book where you lose a level (unless true rez is used) and have XP equal to the middle? Some variant of level loss penalties to checks?

I prefer XP debt equal to the PCs level * 500. The prevent lots of book keeping and does not hose the character that is close to leveling. But it does cause players to end up with level differences, which I like, without causing players to redo their character.

We haven't discused it in my game yet, but I like the idead of XP debt instead of level loss.

One thing to consider is your method of giving out xp. If you are one who just gives everyone the same amount of xp each time, then xp debt is fine. However, if you give lower level PCs more xp than higher level ones, XP debt is less ideal.
