Death Saving Thow bonus's

You get to spend a surge on a 20.

If you have a feat or item that gives you +2 to Death Saving throws.

Do you get the surge on a natural 20.

Or on a 18 to 20 ?

Cleric thinks he is on to a good thing here, I am not sure.

18 to 20. Nowhere does it specify that the result must be natural in that case. In fact, it says "20 or higher." If bonuses didn't count, there would be no need to specify "or higher."

Agreed. The text does not specify that you must roll a natural 20.
