Are you bored with your current campaign or the current group of PC's? Do you feel it needs a radical change of direction or a shake-up amongst the PC's?

Then you should absolutely introduce the bundle of chaos and tears that is the Deck of Many things.
 

Maybe I'll hold off then. My group are level 5, just finishing phandelver then doing a homebrew, all thoroughly invested in their characters. Due to their inexperience (everyone's first campaign) I know if I give it to them and give them very stern warnings, they'll just be like "I draw ten cards" then get really angry when there character loses all their xp, all their items and is teleported to a different plane

Sometimes, people get complacent. And that's when you need to kick them off the couch, set the couch on fire, bulldoze the house, and chase those people, naked, afraid, and penniless across the border into a foreign country where they don't know the language.

Keeps things fun.

Whether you use it is one question - how you use it is another. I would never (again) just drop it into a game for the chaos of it. I'd build a segment of the game around it and have it be the final culminating event of that segment of the game. As for the levels where I'd use it - 9 or above.
