Dimensional Rifts

Anybody seen a project or hacked out a d20 based system for dealing with "dimensional rifts?" I'm wanting some rules for my rifts.

Not exactly sure what you are looking for. If you want ways to incorporate manny DnD worlds I reccomend the following three products: Manual of the Planes, Player's Guide to the Planes and Portals & Planes (from Fantasy Flight Games).
 


I just picked up Portals 7 planes recently and also reccomend it.

It's clear, concise and not setting-specific, which worked in this book.

And though I do not have it, would Monte Cooks' "Beyond Countless Dorrways" be relevant?
(I might be a bit off on the title)

Clarify:

What I'm looking for is the: Magic so strong/terribal/a scientific mistake it/that caused a rip/tear/hole in the time space continuum thing. Aweful things happening/spewing out as a result. Accidental/catastrophic rifts. I'm looking for a formulaized rule-set, to compare to what I've been thinking of doing.

Okay, now I am strongly going to reccomend Portals & Planes for your D20 needs. It does have you covered in that department.
