I believe it started in a town that had been infected via a cistern. I don't recall the name, but I think it had "Crimson" somewhere in the title- maybe someone else can recall more.

I looked into both and discovered Lords of Darkness #2 too in the process which was written for 3e (no diseases but looks interesting).Adam's Wrath had a portion of the module dedicated to dealing with a mysterious disease. Also, the mummy adventure in Lords of Darkness (1e product for FR, I think? Maybe 2e.) had a lot of stuff about getting Mummy Rot.
