DM Performance Review

I'm wrapping up a campaign in a few more sessions and started thinking about some kind of evaluation of the players experience.

I won't be starting another game for a couple of years (everyone in my group DMs so the rotation takes a while) but I'd like some feedback on how this campaign went. I've run other games but never thought to ask the players pointblank about the experience.

Does anyone know of a "DM Performance Review" that I could have my players fill out? If one doesn't already exist, what types of questions would other DMs find useful to have their players answer?

There is an Adventure/DM Review form in Campaign Planner 3. I use this, or a variant, after most adventures, maybe every three or four sessions.

My last survey:
It has been a while since the last survey, so it is time for another. This covers campaigns 1 & 2, the campaign world, and the direction of each. Be honest, even brutally so, as I'm looking to keep the game involved and ever-improving. I promise not to be offended. Please send back your comments to me or to the group, and with your permission I will collect and place them anonymously on the wiki.

1. What have been some of your favorite moments? In other words, what worked for you?

2. What were the worst moments during each campaign, or what sucked?

3. Who is your favorite NPC? Who is the most evil? And who is the most annoying?

4. What magic do you wish to see in the future? Do you wish the PCs could take a break to fashion more items, or would you rather discover & quest for them?

5. What are the most cumbersome rules? How could it improve the game?

6. How could the game improve? Faster rulings? More hand-waving? More between session logistics?

7. What would you like to see more of in future sessions? Butt-kicking fights, puzzles, tactics, undercover/speciali st missions, character interaction, mystery solving, quelling unspeakable horrors? Something else?

8. What would like to see less of in the sessions? Accounting, wilderness, travel, politics, etc.?

9. Is the current level of campaign 1 characters too high? Is campaign 2 too low? Is the feeling of each campaign too disparate? Are they not distinct enough? Would you rather just concentrate on one campaign versus the other?

10. Would you like to drop one of the campaigns in lieu of a 3rd campaign featuring monstrous PCs (using Savage Species supplement) with goals of gathering riches, magic and taking over the world? Or a high-powered, fast advancing campaign something along the lines of www.guildofdefiance .wikispaces. com? Or something else like d20 Modern or d20 Future or d20 <insert genre where book buying isn't required>? Stick with the two campaigns and add the third? Or just stay with the campaigns we have?

11. What would you add (or remove) to the campaign settings? Different cultural settings, new game rules, more/less races, new weapons, unique locations, planar travel, etc.?

12. Any other thoughts, ideas, input?
