It's because my players don't want their PCs to come back from the dead.

It's not that strange. I DM'd a campaign and in one big battle with the BBEG the party's Wiz/Elemental Savant was beheaded (met Mister Vorpal Sword) and the player didn't want to be raised.

I've found that folks who have been playing the game 10+ years tend to leave the character dead.

That's how we're working now. I used to impose a 10% x.p. penalty for replacement characters, but I've since lowered it to 5%.

The players don't complain..although sometimes it sucks to loose a 'really cool' character :)

I voted with the majority (at least for now): 1-3.

In the last several shorter campaigns, up until the most recent, we had no PC deaths (several getting to the negatives, but recovering), but in the last mini-campaign, we lost one PC...twice. Thank goodness somebody could Raise Dead...he almost died again from the poison that killed him initially, after his first raising, but managed the fort save (only barely), and then he got roasted by a mage soon after.
