Grafting is a kind of Item Creation.

Like Item Creation, you need to have the feat, and spent XP and gold on it to create it.
Unlike Item Creation, you need a specific material component: something from a donor creature.
Unlike Item Creation, the resulting Graft can be applied only to one person, that person has to make a sacrifice (hp, dex, etc) and the Graft is then applied permanently.

Other than that, it's just another magic item.

Of course, with magic item creation, if you want to create the base item (sword, shield, boots) you would need the appropriate craft skill.
However, most of the time you just buy the base item (which is covered in the item creation cost)

Since you cannot actually 'create' a living item, (unless you go for construct grafts or Warforged grafts) you cannot create the base item.

Also, IIRC most graft require ranks in 'Heal', so at most appropriate heal checks (when aquiring the material from the donor creature) might be necessary, but this is not covered in the rules as far as I know.
