do monsters get surges and second wind?

The DMG entry isn't a contradiction, so much as it's an exception.

PCs are the only types of things that have Second Wind "unless otherwise noted."
NPCs have Second Wind, as otherwise noted.
Sphinxes have Second Wind, as otherwise noted.

Every rule in 4.0 has the parenthetical 'unless otherwise noted' appended to most blanket rules.

Interesting. That would seem to contradict the text from page 291 of the PHB. I wonder if there's an order of authority for the 4e core rulebooks as there was for the 3.5e books?

Whatever the primary source is for the topic you're looking at. A passing mention in the PHB doesn't invalidate a hardset rule in the DMG.

Correct.

The PHB is not considered by any means any sort of authority for any hard rule involving the creation of non-player anythings.

One healing surge per tier. No second wind unless specified otherwise.

Awesome. Thanks everyone ! I just wasn't able to find it.

I figured some must get a surge (to activate magic items and such) but I wasn't sure about the second wind ability itself.
