Do you ever give a PC the "exact" magic item they ask for?

Whoa. That is harsh. What is wrong with asking for what you want? If you don't ask, you'll never get.

But, kicking someone out just for asking the question?

And how is asking for an item meta-gaming. If I go to a car dealership and ask for a Lexus, they don't tell me I'm meta-living. Now if the player uses information outside of the game to figure out a frost giant is coming up then asks for a flaming longsword, that would be meta-gaming. But saying I would like a flaming longsword because I think they are cool isn't meta-gaming.

I like the idea of turning it into the quest. Any good entertainer (DM) will tell you give the crowd what they want. That doesn't mean give it to them for free.

Incorporate their interests into the game. The DM who doesn't will end up finding himself playing alone.
