There are several causes of babies being larger than average but being more of average size when they get older (for example, if a mother is diabetic during pregnancy, even if she normally is not, that can lead to larger sized embryos and of course, thus in turn, infants).

12 pounds in unusually large (That's 5.4 Kilos...almost 5.5 K).

My wife has type 2 diabetes. As you note that often causes larger than normal babies, but not usually as large as my son was.

He was also born with pulmonary hypertension and spent 6 weeks in the NICU. They had to paralyze him and sedate him with drugs, because any stress or muscle tightening could have been fatal. Once out of danger they had to get him through addiction withdrawals. The poor kid had a tough first several weeks.
