Far Realm Campaign Idea

Hello,
I have a campaign world (Malune) that I've been working on for a couple years now. It serves as the locale for the campaign I run over the summer. The campaign is pretty much standard high fantasy, but I've added a twist. My world is about to fall apart. And I don't just mean wars and such or even the triumph of a dark god. The multiverse itself is going to be reabsorbed into the Far Realm.
According to the cosmology I wrote, the material plane and the astral were created from the primeval world of the Far Realm by a powerful uber-diety. For an unknown reason (I truthfully don't know why) the uber-diety "broke" into three slightly less powerful divinities. This sparked a godswar that nearly destroyed the world but also led to the creation of other planes within the astral. <lots of other history> Now, the multiverse is decaying for some reason and the Far Realm is encroaching on all of the planes.
Where I could use some help is how to introduce this planar decay and ideas for mechanics for running a decaying plane. Also, if any of you have used Far Realm entities before and could offer remarks on them I'd appreciate it.

A disintegrating plane can be approached in a few ways. Here's how I would do it:

The fabric of reality itself is breaking down, so the boundaries between planes are becoming thin and permeable. Portals to random planes spawn chaotically, sucking creatures to unknown extraplanar fates, or ejecting the local contents of what lies on the other side. Larger portals would become increasingly common as the disintegration progressed, capable of ripping away entire landscapes... or replacing those that exist with the contents of other planes.

Planar 'constants' such as the flow of time and the nature of gravity and magic (as described in the manual of the planes) would begin to fluctuate wildly.

As the power of the far realm grows, consider randomly inflicting creatures and individuals with the Pseudonatural template, as found in Tome and Blood and Manual of the Planes (Don't forget the horrific descent into gibbering betendriled insanity).

Additionally, normal physical matter begins to unravel into its constituent elemental components, resulting in elemental detonations and the gradual reduction of reality to soupy writhing chaos (Toward the end, it would be much like Limbo... before merging with the incomprehensible abyss of the far realms.)

As for entities of the far realm, a good starting point is the 'mindset' (I use the term loosely) of creatures encountered in the works of H.P. Lovecraft. Generally speaking, compassionless all consuming madness... with occasional overtones of assimilation into the nightmare, rather than merciful destruction. Aberrations fit into the far realm very well. And again, I suggest liberal application of the Pseudonatural Template to existing creatures. From the RP side of things, try to make pseudonatural equivalents of monsters act like their normal counterparts... but with their behavior and personalities as twisted and disturbing as their physical forms are, when compared to normalcy.

For example, a pseudonatural Solar would still be 'good'... but in a way that was hideous and repulsive to normal creatures. It would be faking its morality for effect, just like its shape. To a pseudonatural being, morality and ethics are as malleable and superficial as physical form.
