Flesh Golem template?

Does anyone have a template for creating flesh golems? Specifically, for turning a living creature into a flesh-golem version of itself?

If not - do you think such a template should have a HD increase?

I see flesh golems as more of a Frankenstein's Monster thing - you're putting together bits and pieces from different creatures, not reanimating one single creature (that's called a zombie). Of course, you could use bigger and/or more bits to get a bigger flesh golem - that's covered by the advancement range. I think I've seen somewhere (Sage advice?) that you just scale up the cost of the golem along with the HD (so if a 10 HD golem costs 10,000 gp, a 15 HD golem costs 15,000 gp).
