Formian Queen, isn't she very vulnerable to CDG?

A helpless character is paralyzed, held, bound, sleeping, unconscious, or otherwise completely at an opponent’s mercy. A helpless target is treated as having a Dexterity of 0 (–5 modifier).

We can tell from the Formian Queen's stat block that she is not treated as having a Dexterity of 0 (–5 modifier); therefore she is not considered helpless; therefore she is not considered otherwise at your mercy.

Looks like we are back at this issue…

If you like. Just like the harpy's victim, the Queen is not helpless, therefore not a valid target for CDG.

Just because she can't move (leave her space) under her own power, doesn't make her helpless. You can't CDG someone who's failed their save vs. Web--the fact that they're stuck (for now) in their current space doesn't stop them from ducking or weaving to avoid blows. The Formian Queen is evidently in this same sort of situation--she's not a motionless blob of chitinous flesh, but the little motion she's capable of doesn't include locomotion, only just enough to turn the would-be decapitation into a nick in the neck.
 

Blur would keep the hit chance down a bit, as would invisibility and displacement.
There's a PrC that would be perfect for this type of creature. I can't remember the name of it offhand, but they can use their minions/lackies/bodyguards/whatever to take hits for them.

Well, then we must read "utter lack of physical activity" differently.
