Actually, that arctic fox is Small, not Tiny. ;)
I'd go with that animal. The tip regarding the DRAGON magazine is good, even if I don't remember which issue it was.

If you do use the Small Dog from the MM, I'd increase its base land speed by 10 feet. In general (I believe) foxes are faster than most small dogs.
 

I played a gnome with a fox familiar, named Red. Used the stats from Dragon 291. This info should be straight out of Dragon, with the exception of the hp total:

Red: Male fox familiar: CR 1/4; Small Animal; HD 1/2d8; hp 10; Init
+4 (Dex); Spd 40 ft.; AC 17 (+1 size, +4 Dex, +2 natural); Atk +1
melee (1d4, bite); SQ scent; Face 5 ft. by 5 ft.; Reach 5 ft.; SV
Fort +2, Ref +6, Will +1; Str 11, Dex 19, Con 11, Int 2, Wis 12, Cha
7. Skills: Hide +12, Listen +5, Move Silently +9, Spot +5, Swim +3.

Hm. If you make a lycanthrope using an animal with 1/2 d8, how many animal HD does the lycanthrope have? One? None? Fractional HD are weird.
