Ghouls have been templated nearly to death, honestly.
In addition to the ones posted above, I beat most of them to the punch with the publication of the ghoul template in Necromancer's Legacy (the template was also originaly published as a freebie PDF on my D&D fansite shortly after 3e was released).
I've also got a Ghoul Paragon class around here somewhere waiting to be released in Necromancer's Legacy 2 - Undead Boogaloo!
